Community Connector

In Colorado, Community Connector services are designed for children and youth with intellectual and developmental disabilities to help them build independence and successfully access resources, services, and activities within their communities. These services focus on increasing community integration, strengthening natural supports, and helping individuals develop the skills needed to navigate community settings safely and confidently. Community Connector services may include assistance with recreational programs, educational opportunities, volunteer activities, social events, and community participation while encouraging communication, self-advocacy, relationship building, and age-appropriate skill development. Services are generally available to eligible youth and young adults through Colorado Medicaid waiver programs, depending on waiver guidelines and individual eligibility requirements.
